Welcome to the HealthSource RI application. In this section, you will create your HealthSource RI account and verify your identity. We verify your identity (determine that you are who you say you are) to protect your privacy. Once your identity is verified, you can apply for health coverage and find options to help pay for insurance.

  • Birth date
  • Social Security Number
  • Health insurance information
  • Income and tax information (such as previous tax returns, pay stubs, W-2 forms, 1099 forms, unemployment benefits, etc.)
  • Immigration or citizenship information
